Canes and Crutches

CRUTCHES, FOREARM, INCLUDES CRUTCHES OF VARIOUS MATERIALS, ADJUSTABLE OR FIXED, PAIR, COMPLETE WITH TIPS AND HANDGRIPS. Canes, crutches, and walkers improve balance, assist walking, reduce load on the lower limbs, transmit sensory cues, and enable maneuvering in places inaccessible to a wheelchair. Assistive devices are manufactured in many designs. Canes differ by material, handle shape, and base. Crutches may have an axillary piece, a forearm cuff, or an upper arm support. Walkers can have wheels or rubber tips. Correct measurement is essential with any assistive device. Gait patterns used with assistive devices may be alternating (reciprocal), in which the user moves one foot at a time, or swinging (simultaneous), with patient moving both feet at the same time. Kinetic analyses indicate that the duration and amount of force applied to a device varies according to its purpose: balance, pain relief, or transferring the body through space. Kinematic analyses confirm that patients tend to walk faster when aided by a device. Heart rate and energy consumption are affected by the type of device and the way it is used. Benefits associated with device use include greater stability and reduced stress on the lower limbs. Assistive devices may, however, compress nerves in the upper limb and may embarrass the user.

Commodes/Urinals/Bedpans

Recovering From Surgery: Individuals who are bedridden or recovering from surgery or injury can benefit from a bedside commode. The medical commode allows individuals to easily slide into the toilet from their bed, with limited or no assistance. Individuals in Wheelchairs: Drop arm commodes are an aid for those in wheelchairs. With the ability to collapse one arm, an individual can easily slide into the commode without assistance from another. Bedside urinals allow individuals to relieve themselves without stepping out of bed. They are designed to be lightweight and to minimize stillage. Bed pans are used to collect urine or feces. They are shaped to fit under a person lying or sitting in bed. They can be made of plastic or metal, and some can be used with liners to prevent splashing and to make cleaning easier

Enteral Nutrients

Enteral nutrition is covered under the Prosthetic Device benefit (Social Security Act § 1861(s)(8)). In order for a beneficiary's nutrition to be eligible for reimbursement the reasonable and necessary (R&N) requirements set out in the National Coverage Determinations (NCD) Manual (CMS Pub. 100-03), Chapter 1, Section 180.2 must be met. In addition, there are specific regulatory, payment and coding requirements, discussed below, that also must be met. Enteral nutrition products that are administered orally and related supplies are noncovered; no benefit. If the coverage requirements for enteral nutrition are met, medically necessary nutrients, administration supplies and equipment are covered. Enteral nutrition provided to a beneficiary in a Part A covered stay must be billed by the Skilled Nursing Facility (SNF) to the A/B MAC. No payment from Part B is available when enteral nutrition services are furnished to a beneficiary in a stay covered by Part A. However, if the stay is not covered by Part A, enteral nutrition may be eligible for coverage under Part B and may be billed to the DME MAC, by either the SNF or an outside supplier.

A pressure reducing support surface is designed to prevent or promote the healing of certain types of pressure ulcers by reducing or eliminating tissue interface pressure. Pressure injuries (pressure ulcers, pressure sores) are areas of skin and tissue damage that form in regions with unrelieved and/or increased pressure. Pressure injuries are often associated with prolonged immobility, poor nutrition, and/or comorbid medical conditions. There are many pressure-reducing support surfaces used to care for and prevent the development of pressure injuries, including specialized mattresses and mattress overlays. The primary function of these support surfaces is to prevent focal areas of pressure and to better distribute the body surface over a larger area. Pressure-reducing support surfaces are categorized into three groups based on their features, such as the type of material and whether they are powered by electricity or not.

Insulin Infusion Pumps

EXTERNAL AMBULATORY INFUSION PUMP, INSULIN. The long-term complications of diabetes mellitus remain a serious public health problem in the Western nations. Much current research focuses on methods of insulin treatment designed to provide near-normal glycemic control throughout the day with the intention of preventing or delaying the onset of the angiopathic consequences of preventing or delaying the onset of the angiopathic consequences of diabetes. Recent work has shown that open-loop insulin infusions (that is, insulin administered by an infusion pump without feedback control) can result in near-normal blood glucose levels in juvenile-onset or Type I diabetics. The extracorporeal infusion pumps used in such studies have their won physical and psychological drawbacks, however. More recent studies have attempted similar degrees of control using totally implantable insulin infusion pumps; several such studies are being carried out in patients with adult-onset or Type II diabetes, the predominant type of diabetes afflicting the elderly.

Orthoses: Penile Pumps

A penile prosthesis is another treatment option for men with erectile dysfunction. These devices are either malleable (bendable) or inflatable. The simplest type of prosthesis consists of a pair of malleable rods surgically implanted within the erection chambers of the penis. With this type of implant the penis is always semi-rigid and merely needs to be lifted or adjusted into the erect position to initiate sex. This type of implant is a good choice for men with spinal cord injuries and/or limited hand strength. Today, many men choose a hydraulic, inflatable prosthesis, which allows them to have an erection when they choose, and it's easier to conceal. It is also more natural. A penile implant is usually used when there is a clear medical cause for ED and when the problem is unlikely to resolve or improve naturally or with other medical treatments. Sometimes a penile prosthesis is implanted during surgery to reconstruct the penis when scarring has caused erections to curve (Peyronie's disease) or to treat severe cases of recurrent prolonged erections (priapism). Penile implant surgeries take about an hour and are typically done in an outpatient center. A man can resume sexual intercourse by 6 weeks after surgery.

Osteogenesis Stimulators

Osteogenesis stimulators, also known as bone-growth stimulators, apply an electric current or ultrasound to the spine or a long bone (e.g., the femur) and are used when a fusion or fracture failed to heal or after a multilevel spinal fusion. An ultrasonic osteogenesis stimulator will be denied as not medically necessary if any of the criteria above are not met. Use of an ultrasonic osteogenesis stimulator for the treatment of a fresh fracture or delayed union will be denied as not medically necessary. Ultrasound conductive coupling gel is covered and separately payable if an ultrasonic osteogenesis stimulator is covered. An ultrasonic osteogenesis stimulator will be denied as not medically necessary if it is used with other noninvasive osteogenesis stimulators.

Patient Lifts

Patient lifts are designed to lift and transfer patients from one place to another (e.g., from bed to bath, chair to stretcher). These should not be confused with stairway chair lifts or elevators. Patient lifts may be operated using a power source or manually. The powered models generally require the use of a rechargeable battery and the manual models are operated using hydraulics. While the design of patient lifts will vary based on the manufacturer, basic components may include a mast (the vertical bar that fits into the base), a boom (a bar that extends over the patient), a spreader bar (which hangs from the boom), a sling (attached to the spreader bar, designed to hold the patient), and a number of clips or latches (which secure the sling). These medical devices provide many benefits, including reduced risk of injury to patients and caregivers when properly used. However, improper use of patient lifts can pose significant public health risks. Patient falls from these devices have resulted in severe patient injuries including head traumas, fractures, and deaths.

Pneumatic Compression Devices

Intermittent pneumatic compression (IPC) devices are used to help prevent blood clots in the deep veins of the legs. The devices use cuffs around the legs that fill with air and squeeze your legs. This increases blood flow through the veins of your legs and helps prevent blood clots. Veins are the blood vessels that bring oxygen-poor blood and waste products back to the heart. Arteries are the blood vessels that bring oxygen-rich blood and nutrients to the body. A deep vein thrombosis (DVT) is a blood clot that forms in a vein deep inside the body. In most cases, this clot forms inside one of the deep veins of the thigh or lower leg. The veins in your legs have tiny valves that help keep blood moving back up toward the heart. But a DVT may damage one or more of these valves. This causes them to weaken or become leaky. When this happens, blood starts to pool in your legs. This can also happen if you are immobile for a long period of time. Normally, muscles in the leg help blood move up in the veins when the muscles contract. When blood flows very slowly through the veins, this increases the risk that cells in the blood will stick together and form a clot. DVT is a common condition, especially in people over age 65. Post-thrombotic syndrome affects a large number of people who have had DVT. It can happen in men and women of any age. DVT is a serious medical condition that can cause swelling, pain, and tenderness in your leg. In some cases, a deep clot in a leg vein can break free and stick in a vessel in the lung. This can cause a blockage in the vessel called a pulmonary embolism. Pulmonary embolism can cause severe shortness of breath and even sudden death. While using an IPC device, your calf or whole leg is enclosed in a cuff. The cuff fills with air and squeezes the leg, much like a blood pressure cuff. Then the cuff deflates and relaxes. The process then repeats over and over. The compression helps move blood through your veins towards your heart. IPC also promotes the natural release of substances in your body that help prevent clots. Between compressions, the cuffs of the device relax, and oxygen-rich blood continues to flow in the arteries of your leg.

Power Operated Vehicles (Scooters)

A mobility scooter is an electric vehicle and mobility aid mostly auxiliary to a power wheelchair but configured like a motor scooter. When motorized they are commonly referred to as a power-operated vehicle/scooter, or electric scooter. Non-motorized mobility scooters are less common, but are intended for the estimated 60% of wheelchair users who have at least some use of their legs. Whilst leg issues are commonly assumed to be the reason for using scooters, the vehicles are used by those with a wide range of conditions from spinal to neurological. Mobility scooters differ from power wheelchairs in that they are usually cheaper, somewhat easier to move across uneven ground, and are more customizable. These scooters are built for people who have trouble walking or getting around, but don't need a power wheelchair. They are also used by people who need a powerchair for intermediate distances or extended standing. Some people use mobility scooters because they are not allowed to drive cars for medical reasons.

Seat Lift Mechanisms

Seat lift mechanisms are a type of assistive device used to lift the body from a sitting position to a standing position. The mechanism is also capable of lowering the patient from a standing to a sitting position. Seat lifts are generally recommended only for patients who are able to ambulate once they are standing.

Speech Generating Devices

Speech-generating devices are hand-held electronic devices that play words or phrases when the user touches a switch or presses buttons or keys. Some devices 'speak' words as the words are typed on a keyboard. Speech-generating devices can be specialised devices used only for communication.

Surgical Dressings

A dressing is a sterile pad or compress applied to a wound to promote healing and protect the wound from further harm. A dressing is designed to be in direct contact with the wound, as distinguished from a bandage, which is most often used to hold a dressing in place. Many modern dressings are self-adhesive.

Transcutaneous Electrical Nerve Stimulators

Transcutaneous electrical nerve stimulation (TENS or TNS) is the use of electric current produced by a device to stimulate the nerves for therapeutic purposes. TENS, by definition, covers the complete range of transcutaneously applied currents used for nerve excitation although the term is often used with a more restrictive intent, namely to describe the kind of pulses produced by portable stimulators used to reduce pain.[citation needed] The unit is usually connected to the skin using two or more electrodes which are typically conductive gel pads. A typical battery-operated TENS unit is able to modulate pulse width, frequency, and intensity.
